Am I responsible for his medical debt?

Once you legally separate, request to remove him from your insurance. If you don’t, the court might order you to keep him on it until the divorce is done.

It doesn’t really. Financial and legal separation is part of the divorce process. You file for divorce, then work out financial details and custody. Once those are settled, the divorce is finalized. If you want separation without divorce, you can, but it’s usually part of the process.

Not a lawyer, just someone with experience with Medicaid and divorce. Medicaid should cover his medical bills, but if prior authorization was needed and wasn’t done, you could be responsible for that debt. It depends on the rules in your area. I’d recommend talking to a lawyer to be sure.
